Yep, was thinking the same thing.
PS, have you mentioned this before?
I didn't mention it as I'd get lambasted for it adding extra cost, weight, drag, decrease in mpg,etc.

BTW: Where did this 84mpg figure come from?
From real world testing?, or just wishful thinking?
If they didn't want to add a second hump (which I think would balance out the design), they could get real clever and redesign slightly the side door bracing on both sides as dual purpose bracing/ventilation conduits to also carry air back to the passenger. That wouldn't add to the exterior and shouldn't be much of a weight penalty, either.

Why would they do that when they have a side that does not have a door. I am sure it would be much easier to run it there.

Most new cars have vents under the front seats to pipe HVAC to the rear seat passengers. I even have these in my 2008 Dodge Nitro, and had them in my 99 Monte Carlo.
The Luxury cars will even have a separate set of controls for the HVAC to the back seat passengers.

There is a thread on the site where we covered this topic in detail.
Including an idea to mount a headrest monitor in the driver seat and have the option to connect it to the forward view camera.
Getting out was covered with a number of possibilities.

Ok the driver is "incapacitated" as in knocked out, or D-E-A-D, how is rear seat passenger getting out?

Are rear windows the type that open, or are they fixed (Glued in).
For safety reasons, opening rear windows should be STANDARD as they can in a accident be pushed/pryed open/removed.

Q: Are rear opening windows standard?
Q: Is there enough room for a adult, to fold down rear seat so they could exit via the hatch?

All two door cars are like that. I think, by design the seat release is accessible to the back seat people for that reason.
As far as the comfort of an Elio passenger is concerned. I think they may be thinking about that too.

Yep, all two door vehicles these days have releases that would allow rear passenger(s) to ESCAPE, but the BIG difference is that in a regular car rear passenger(s) have a heck of a lot more room to maneuver, and it is unlikely both persons would be incapacitated.
Even if both were dead, typically persons could crawl between the seats to freedom.

So, Are opening rear windows standard?

What about the rear hatch?

Rear opening glass comes in handy not only for ventilation but for those times in which hatch is jammed from impact, or person in seat is too big, or can't crawl over seat...or can't fold down the seat.
(Seat only folds forward, right?)
